2026–2027 Medical Requirements

Dear Parents/Guardians:
We hope this letter finds you and your family well. In preparation for your child’s upcoming school year,
please ensure all of your child’s health information is current in the Nurse’s office. Below you will find a
summary of important health information and pertinent documents.
Medical forms and information can also be found on the Laurel Hill Website Laurel Hill School (click on
parent portal).
Documents must be completed and uploaded to Finalsite by August 21, 2026
This will allow the school nurse time to review all documents prior to the start of school.
Documents submission after August 21st will need to be submitted in person by a parent/guardian.
Students who are not in compliance with the Department of Health regulations will be excluded from school on Tuesday, September 16, 2026.
Physical Exams: New York State (NYS) law requires a health examination for all students entering a school district for the first time and when entering Pre-K or K, 1st, 3rd, 5th, and 7th grades. Exams are valid 12 months prior to the first day of school. All exams must be documented on the Required NYS School Health Examination Form. Please submit this form within the first 30 days of school. It must be dated after September 7th, 2025.
Immunizations: Students are required to be compliant with New York State immunization requirements in order to attend school. Parents of students entering Pre-K, K, 6th, 7th grade should be aware of additional specific grade level requirements.
Additional Requirements
Screenings
NYS mandates health screening for vision hearing, and scoliosis. If the screening information is not
completed on the school health form, the School Nurse will perform mandated screenings during the school year.
Medications
Any medications required during the school day must be documented on a Medication School Form and signed by BOTH the physician and the parent/guardian. All medications must be in their original bottle or packaging. Students are not permitted to carry/bring medications to the health office. All medications must be brought in by a parent/guardian.
Medicine Administration Form: Medicine Administration
Special Needs / Medical Conditions
If your child has special needs, allergies, or other medical conditions, please notify the school health office so an appropriate plan of care can be put in place.
Medical Care Plans: Asthma, Allergy/Anaphalaxis, Seizure, Diabetes
If your child has a medical care plan in place such as asthma, allergies, seizures and /or diabetes and requires medication and or an emergency care plan, please complete the form below.

It is important to call your family physician early to schedule an appointment and avoid delays. Please feel free to contact the health office at 631-751-1154 ext. 2 with any questions or concerns.
